ข้าพเจ้าให้ความยินยอมในการเก็บรวบรวม ใช้ และเปิดเผยข้อมูลส่วนบุคคลของข้าพเจ้า เพื่อนำข้อมูลไปใช้สำหรับ โครงการเดิน - วิ่ง มินิมาราธอน เพื่อทหารผ่านศึก "เสียเหงื่อ เพื่อผู้เสียสละ" (WVO RUN FOR HERO 2025) เท่านั้น

คำรับรอง/การยินยอมของผู้สมัคร

1.ข้าพเจ้ารับทราบว่าการเข้าร่วมแข่งขันวิ่ง ซึ่งเป็นกิจกรรมที่มีความเสี่ยง ซึ่งอาจก่อให้เกิดการบาดเจ็บหรือเสียชีวิต ข้าพเจ้าตกลงที่จะไม่เข้าร่วมกิจกรรมหากไม่สามารถเข้าร่วมได้ด้วยเหตุผลทางการแพทย์หรือไม่ได้รับการฝึกฝนอย่างถูกต้อง

2.ข้าพเจ้าตกลงที่จะปฏิบัติตามการตัดสินใจของคณะกรรมการ ผู้จัดการแข่งขันที่เกี่ยวเนื่องกับความสามารถของข้าพเจ้าในการเข้าร่วมแข่งข้น

3.ข้าพเจ้าขอรับรองว่า ข้าพเจ้านั้นมีสุขภาพที่ดีและได้รับการฝึกฝนสำหรับระยะทางที่จะเข้าร่วมแข่งขันและถือว่าความเสี่ยงทั้งหมดที่เกี่ยวข้องกับการแข่งขันกิจกรรม ซึ่งรวมถึงแต่ไม่จำกัดเฉพาะการล้ม การสัมผัสกับยานพาหนะหรือผู้เข้าร่วมแข่งขันบนผลกระทบจากสภาพอากาศ ซึ่งร่วมถึงอุณหภูมิความร้อนและ/หรือความชื้นสูงการจราจรและสภาพถนน

4.ข้าพเจ้ารับทราบและเต็มใจรับผลกระทบความเสี่ยงทั้งหมดนี้ จากการที่ได้อ่านทำความเข้าใจและรับทราบข้อเท็จจริงในเอกสารนี้ ข้าพเจ้าตกลงที่จะสละสิทธิ์และละเว้นในการเรียกร้องค่าเสียหาย จากทั้งคณะกรรมการผู้จัดการแข่งขัน "WVO RUN FOR HERO 2025" ร่วมถึงผู้สนับสนุนการแข่งขันบุคคลากรทุกฝ่าย ตัวแทน เจ้าหน้าที่ ผู้อำนวยการ สมาชิกและผู้รับจ้างทำงานของการแข่งขัน

5.ข้าพเจ้าจะละเว้นในการเรียกร้องค่าเสียหายหรือหนี้สิ้นทั้งในปัจจุบันและอนาคตอันเนื่องมากจากการเข้าร่วมกิจกรรมถึงแม้ว่าความเสียหายนั้นอาจเกิดจากความประมาทหรือความไม่ระมัดระวังในส่วนของบุคคลที่กล่าวอ้างถึงข้างต้นในเอกสารนี้ในกรณีที่กิจกรรมนี้ต้องยกเลิกทั้งหมด เลื่อนการจัดงานหรือส่วนใดส่วนหนึ่งโดยสืบเนื่องมาจากสาเหตุสุดวิสัยใดๆ เหตุจากธรรมชาติหรือภาวะอื่นใดก็ตาม ข้าพเจ้ารับทราบและยินยอมว่าจะไม่มีการคืนเงินค่าสมัครให้แก่ข้าพเจ้า

6.ข้าพเจ้ายินดีให้สิทธิ์ผู้จัดงานในการใช้ภาพนิ่ง ภาพเคลื่อนไหวการบันทึกทุกรูปแบบจากการเข้าร่วมกิจกรรมสำหรับวัตถุประสงค์ที่ชอบด้วยกฎหมายโดยไม่มีการเรียกร้องค่าตอบแทนแต่อย่างใด

I know that running is a potentially hazardous activity, which could cause injury or death. I should not enter and run unless I am medically able and properly trained. 

I agree to abide by any decision of a race official relative to my ability to safely complete the run. 

I hereby certify that I am in good health and I have trained to run the distance of the race, which I am entering. I assume all risks associated with running in this event including, but not limited to: falls, contact with other partici-pants, the effects of weather, including high heat and/or humidity, traffic and the conditions of the road, all such risks being known and appreciated by me. Having read this waiver and knowing these facts and in consideration of your accepting my entry into this running race, 

I for myself and anyone entitled to act on my behalf, waive and release "WVO RUN FOR HERO 2025", Inc., its officers,directors, agents, volunteers and employees, all states, cities, counties, the Metropolitan District Commission or other governmental bodies or locations in which events or segments of events areheld, all sponsors, their representatives and successors, from all claims or liabilities of any kindarising out of my participation in this event even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver. 

I hereby agree that in the event of the event being cancelled in whole or in part due to wind, storm, rain inclement seas or weather orother condition, my entry fee shell not be refundable. 

I grant permission to all of the foregoing to use any photographs, motion pictures, recordings, or any other record of this event for any legitimate purpose.
